The problem
2/8 + 3/8 comes out as 5/16
Children treat a fraction as two separate whole numbers and add the tops and the bottoms, because adding is all they have practiced.
Adding the denominators too
The bottom number names the size of the pieces. Eighths plus eighths are still eighths, but many children write 16 anyway.
Top and bottom feel unrelated
If a fraction is just two stacked numbers, it is hard to see why only one of them changes when you add.
Whole-number habits are strong
Adding 2 + 3 on top and 8 + 8 on the bottom feels like one job. It takes a few worked examples to show that the pieces do not change size.
Each question keeps the pieces the same size and asks only how many there are. The wrong answer choices are the exact traps, such as 5/16, so a slip shows up and gets talked about.
